Information-Sharing Networks
NYC Department of Health & Mental Hygiene Navigator Program Network. The Navigator Program Network (NPN) brings together case workers, practice managers, physicians, nurses, administrators, navigators, and others working towards the common goal of saving lives by making cancer screening an easier process. Starting with colon cancer screening, NPN facilitates the flow of information and ideas.
Community Health Worker Network of NYC. Unites CHWs to share experiences and resources, inform policy issues and guide development of the CHW field.
CHW/Patient Navigator Workforce Studies
Patient Navigator Study.
The purpose of this study is to gather information on the daily
activities and characteristics of patient navigators across the country
in order to better inform the public and the healthcare field about the
important work navigators accomplish.
Community Health Worker National Workforce Study.
A comprehensive national study of the community health worker workforce
and the factors that affect is utilization and development.

Workshops are for Trainers, Health Educators and supervisors of Outreach units.

NYU Langone Medical Center’s Diabetes Education Center. Teaches diabetes self-management and self-care skills.
NYC Department of Health and Mental Hygiene, Bureau of Chronic Disease Prevention and Control Training Programs
- Asthma Training Institute: Offers a calendar of asthma workshops that cover key areas of knowledge and skills. Special workshops are also offered that address best practices, emerging concerns and current knowledge on a variety of asthma-related topics.
- Cancer Patient Navigator Program Training Program: This five-day training covers best practices for working toward the goal of increasing colonoscopies, community outreach skills, and how to help patients navigate the health care system and overcome barriers that keep them from getting a colonoscopy. - Primary Care Nutrition Training: Discusses key nutrition messages that can be given to patients during the primary care visit. Contact: Saimone Walker, MPA (212) 676-2044 or swalker4@health.nyc.gov.
- Self-Management Support Through Goal Setting: Teaches providers and support staff to provide multi-disciplinary self- management support to patients with chronic diseases. Important aspects of self management which this course teaches providers to both experience and help patients with includes: goal setting, enhancing self-efficacy and effective communication skills. - Taking Charge of a Chronic Disease: An Introduction to Self-Management:This three session workshop will introduce participants to key concepts in chronic disease self management, provide practice in skills that help support effective self management and explore how participants can apply these skills in supporting people with a chronic disease. Research on CHWs
The Columbia University Mailman School of Public Health has conducted a study utilizing CBPR that engaged CHWs to get their perspective on the definition and role of CHWs, training needs, and certification preferences.
